1. I/We have read Liberty’s Data Privacy Statement and express my consent to the processing of my personal information, which includes but is not limited to, the collection, recording, organization, storage, updating or modification, retrieval, consultation, use, consolidation, blocking, erasure or destruction of my personal data. In accordance with Republic Act No. 10173, otherwise known as Data Privacy Act of 2012 and its Implementing Rules and Regulations.
2. I/We agree that my/our application, enrollment, purchase, maintenance, access or continued use of any of Liberty’s products and services shall be deemed as my/our acceptance and agreement to be bound by the provisions of Liberty’s Data Privacy Statement.
3. I/We agree that the Data Privacy Statement shall in all respects apply to my/our application, account and any matter arising therefrom or incidental thereto. I/We agree that the Data Privacy Statement is deemed to be incorporated by reference into this Consent.
4. I/We acknowledge that Liberty may collect, record, organize, store, use, process, update, modify, retrieve, and share my personal information to its employees, duly authorized representatives, agents and brokers, other insurers, reinsurers, adjusters, investigators, and other third party providers for purposes provided in Liberty’s Data Privacy Statement and compliance with legal, regulatory or contractual requirements
5. I/We understand and consent that the processing of my personal information may extend for a period of at least five (5) years from the termination of policy, account, service or transaction, and that I/ we understand and consent that the storage may extend beyond the said period should there be any requirement or disclosure obligation, dispute or action until the final conclusion thereof.
6. These provisions shall be in addition to, and not in substitution for, any other provision agreed to between Liberty and me/us (whether before or after the date hereof) which gives broader rights of disclosure to Liberty than contained herein.
7. I/We hereby affirm my/our right to be informed, object to processing, access and rectify, suspend or withdraw my personal data, and be indemnified in case of damages pursuant to the provisions of the Republic Act No. 10173 of the Philippines, otherwise known as the Data Privacy Act of 2012 and its corresponding Implementing Rules and Regulations.
